CLIENT RELEASE AND INFORMED CONSENT FORM PLEASE READ THE FOLLOWING INFORMATION AND ACKNOWLEDGE THAT YOU UNDERSTAND AND ACCEPT ALL PROVISIONS BY PROCEEDING WITH BOOKING BELOW

SALON POLICIES:

  • NO CHILDREN UNDER 14 ALLOWED IN TANNING ROOMS, NO EXCEPTIONS. Children older than 14 but under age 18 must have written parental consent before using a tanning bed.

 

  • NO CHILDREN UNDER 14 TO BE LEFT UNACCOMPANIED IN THE SALON.

 

  • ONE PERSON PER ROOM, NO EXCEPTIONS.

 

  • ALL PUBLIC AREAS WHERE LEGAL ARE UNDER MONITORED SURVEILLANCE TO THE FULL EXTENT OF THE LAW (EXCEPTIONS ARE RESTROOMS & TANNING ROOMS) FOR YOUR SAFETY.

 

  • WE RESERVE THE RIGHT TO REFUSE SERVICE BUT WE REALLY DON’T WANT TO EXERCISE THAT RIGHT UNLESS WE HAVE TO.

 

  • AS A GENERAL PRACTICE WE DO NOT OFFER REFUNDS ON SERVICES OR PRODUCTS UNDER ANY CIRCUMSTANCE.

 

  • Customers are NOT allowed to exceed the maximum allowable time posted on the tanning device.

 

  • Memberships and Packages (Series Visits) CANNOT be shared. Due to State & Federal Laws, only one person is allowed to tan on an account at a time so that the dates & times of each tan by that specific person may be tracked as required by law.

We accept walk-ins if only we have a room available. Appointments booked online take priority over walk-ins. We encourage everyone to book their session online at www.contourtanbar.com, this guarantees your session time.

Our last appointments are booked NO LATER than 15 minutes before closing time. Walk-ins are not accepted past this time.

SUN BED TANNING:

It is our intention to keep you as well informed about tanning as possible. This means informing you how to operate the tanning equipment. The proper procedure to follow in the tanning room will be clearly explained by a member of our staff. Please feel free to ask any questions. 

IF YOU DO NOT DEVELOP A TAN OUTDOORS, YOU ARE UNLIKELY TO TAN FROM THE USE OF ANY TANNING DEVICE. 

1. AVOID OVEREXPOSURE. As with natural sunlight, overexposure can cause eye and skin injury and allergic reactions. Repeated Overexposure may cause photo aging of the skin, dryness, wrinkling and in some instances skin cancer. We recommend that you do not tan outdoors on days you are tanning indoors, that you do not tan if you currently have a sunburn and that you, at most, tan only once in a 24 hour period. 

2. CERTAIN MEDICATIONS, Lotions and other Products may cause your skin to be more sensitive to UV Rays. Check the posted list of drugs and products known to increase the photosensitivity of the skin. Check with your physician or pharmacist if you are unsure about any medications you are taking or if you have had a problem with indoor or outdoor tanning in the past. 

3. WEAR PROTECTIVE EYEWEAR. Failure to wear protective eyewear may result in severe bums or long-term injury to injuries to the eyes. I have read the contents of this consent form carefully and state that I am not aware of any medical condition or other reason that would prohibit me from tanning. I understand that I will not be allowed to exceed the maximum allowable time posted on the tanning device. I have been given adequate instructions for the proper use of the tanning equipment, understand the risks involved, and use it at my own risk. I hereby agree to release the owners, operators and manufacturers from any damages that I might incur due to the use of this facility.

SUNLESS SPRAY TANNING:

1. A VersaSpa tan will not prevent your skin from burning in the natural sun or in a tanning bed. A VersaSpa tan does not contain sunscreen and does not protect against sunburn. Repeated exposure of unprotected skin while UV tanning

may increase the risk of skin aging, skin cancer and other harmful effects to the skin. Please take the proper precautions when tanning with UV light exposure.

2. For best results, wait 4-6 hours after spray tanning before showering or engaging in any activity that will cause you to perspire. This will allow your tan to fully develop and will ensure the longest results.

3. The VersaSpa booth is designed to provide as even a tan as possible. However, users will be different heights and shapes, and may stand in different positions. It is possible you will find some areas of your tan lighter than others. In most cases, these differences are minor and disappear as your tan fades naturally.

4. More than one VersaSpa session per 12 hours is not recommended.

5. All of the components of the VersaSpa tanning solution have been used in cosmetics for decades and have proven to be safe for the skin. If you have ever had any adverse effects utilizing self-tanning products or moisturizers, consult a healthcare professional before using the product.

6. The FDA has approved DHA (the active ingredient in sunless spray tanning) for external application to the skin. For misting applications, the FDA recommends protective measures to prevent eye contact, inhalation or ingestion. To ensure complete protection, the FDA suggests wearing protective eyewear, nose filters, ear plugs, lip balm and undergarments.

7. VersaSpa sunless spray tanning solution which includes a cosmetic bronzer is water soluble and will be removed after the next wash in cold water. However, in some instances, the DHA in the tanning solution may permanently stain undergarments worn during the spray session.

8. Tanning solution cannot penetrate hair follicles but many customers wear disposable hair nets (provided by the salon) to keep the solution from getting in their hair. There is no evidence that the tanning solution can change hair color.

9. If you have any history of asthma or a respiratory condition consult a physician before spray tanning and do not use if you have had any past allergic reactions to DHA products.

10. I, the undersigned, understand and will comply with all instructions for proper use of the VersaSpa spray tan service. I am using these services at my own discretion.

CANCELLATION POLICY:

Upon booking an appointment and providing your credit card details you are agreeing to our cancellation policy. Nothing will be charged to the credit card in advance of your scheduled service unless the cancellation policies outlined below are violated. You may pay for your service in studio using another form of payment other than the credit card held on file to secure your appointment time.

    • Should you have to cancel less than 1-hours before the start of your appointment, a charge for 50% of your scheduled service(s) will be administered to your credit card on file. If outside salon hours, our voicemail is available!
    • In the event of a ‘no show’ for your appointment time without notifying the salon a charge for 50% of the amount of your scheduled service(s) will be administered to your credit card on file. If outside salon hours, our answering machine and email are always available! We understand things happen that may be out of your control, but please understand we have held that appointment time specifically for you.
    • Please note: you are be able to make modifications to your appointment online through our booking system up until 2-hours before your appointment time.
  • If you arrive more than 10 minutes late without notice you are forfeiting your scheduled appointment time and may have to wait to be seen. Why? Our appointments are only 20 minutes long, so if we agree to take you 10 minutes late we will most likely run late with every single other client for the remainder of the day/night. We understand that things come up and always try do our very best to accommodate you!

I hereby relieve and hold them harmless from any liability involved in the use of the tanning process. The salons and their agents or employees are not liable for any injury to person or property or the loss or theft of any personal property. I know that this facility does not carry liability insurance for injuries caused by tanning devices. I will not tamper with the tanning device. I have been made aware that the salon reserves the right to cancel package without reimbursement for customers who are verbally abusive, act in an inappropriate behavior, do not adhere to the salon rules, and/or act in a destructive or harmful manner. I understand that packages are sold on a per person basis and are not sharable or transferable.
